ADA Complaint Procedure

The American’s with Disabilities Act of 1990 (ADA) prohibits discrimination and ensures equal opportunity and access for persons with disabilities.

Rockford Paratransit is committed to ensuring that no person is excluded from participation in or denied the benefits of its services on the basis of disability if deemed eligible for our service thru the certification process.

Any person who believes that he or she has been subjected to discrimination under the ADA on the basis of their disability may file an ADA complaint with Rockford Paratransit in the following manner:

By Mail:  Please print out and complete this complaint form and mail to 520 Mulberry Street, Rockford, IL  61101

NOTE:  A person may also file a complaint directly with the Federal Transit Administration at FTA Office of Civil Rights, 1200 New Jersey Ave, SE, Washington, D.C. 20590.
